East Lyme Aquatics and Fitness Center (“ELAFC”)
Electronic Funds Transfer (“EFT”) Agreement
ELAFC Membership is an Electronic Funds Transfer (“EFT”) auto-renewal membership that will be automatically charged to the credit card on file associated with the Membership.
Dues will be automatically billed monthly, starting the date this agreement is signed, and continuing on the same day of the month every month, until which time you notify us that you would like to terminate your membership.
In the instance your monthly draft is returned for non-payment, you will be notified via email and phone. If no payment is received by the end of that month, your membership will be terminated and a $25 fee for returned monthly draft charges will be applied to your account. All fees must be paid before Membership can resume.
You may cancel or pause your Membership and the continued billing dues by submitting a cancellation form on the East Lyme Public Schools website under “District” then “East Lyme Aquatics and Fitness Center” then “Memberships.” (https://www.eastlymeschools.org/district/aquatics/membership) A cancellation notice submitted 15 business days prior to your next billing date will result in no further dues being charged.
To the extent permitted by law, you authorize ELAFC to initiate separate EFT/CC charges from the account identified and/or any replacement account for the following amounts: (1) your recurring billing dues; (2) a return fee up to $25 for any EFT charge returned unpaid or on a credit/debit care return; (3) any other fee for goods/services provided, as instructed and authorized by you at that time (ie. swim lessons).
You will receive notice if your EFT membership rate is changing, and will be provided at least 30-days to decide whether to continue your membership before the new rate is charged.
By signing the Membership agreement, you acknowledge you are of legal age, and agree to ELAFC charging your card on file monthly and all other stipulations herein.
